Rules:

Submit your receipts dated 11/10/17-12/15/17 to receive one (1) $10 Navy Exchange gift card when you purchase $25 of eligible Johnson & Johnson products in one (1) transaction from the Navy Exchange. Sales tax is not included towards $25 in purchased products.

Qualifying purchases must be made November 10, 2017 through December 15, 2017 and indicated in one (1) original cash register receipt from the Navy Exchange. (If an additional purchase of $25 worth of qualifying products is made and captured on a separate receipt, then an additional entry may be submitted.)

You have until January 15, 2018 to submit your rebate.

To receive your gift card, please do the following:

1. Complete this form
2. Upload your original Navy Exchange identified cash register receipt dated November 10, 2017 through December 15, 2017 showing a purchase of $25 or more of eligible Johnson & Johnson products.

If receipt does not specify product names, write the last five (5) digits of the product UPC numbers in the space below.

Limit one (1) entry per receipt. You may enter another receipt as a separate entry.
